Because Trump is far to public for this to work. Not like a public figure but between his rallies and press events where he sits sleeping behind the desk in the Oval Office it would not work as well.

Right? This guy’s move right after nearly dying of COVID was to go out on the White House balcony and mouth-breathe in front of a camera crew so everyone could see how “tough” he was.
No way could he actually die without people noticing immediately.
I’d bet he’s literally hooked to machines 24/7 and a doctor or a “doctor” embedded in his entourage. Blood pressure or heart rate takes a slight dip or bump and he could get instant drugs to counteract. Basically the most immediate care anyone could get without being actively attended in a hospital.
